BHI is currently surveying NSW patients. Your experiences in NSW hospitals are important, and your opinion has the power to change how health services are provided to patients in the future.
While participation is voluntary, if you receive a survey, it’s your opportunity to tell us what you think worked for you and what could have been done better. Your feedback is important and will be used to help improve NSW hospital services in the future.
Completing the survey takes about 15–20 minutes. You can complete it online or fill out the paper questionnaire and post it back using the addressed envelope provided. If you have a computer and internet connection, we recommend completing online as this can be quicker and easier than completing the paper version.

To complete the survey in a language other than English, contact the Health Care Interpreter Service on 1800 674 994 (Monday–Friday, 8.30am–5.00pm). A translation sheet is available here.
